Position Overview : You’ll engage with clients facing a range of challenges, offering therapeutic interventions and connecting them with resources that enhance their well-being.
This role is about more than just assessments it's about seeing each person as a unique individual and helping them achieve meaningful change.
Salary : $110,000 - $130,000 per year +full benefits
Key Responsibilities :
- Perform in-depth psychosocial assessments to identify client strengths and needs.
- Create personalized treatment plans and provide ongoing support.
- Collaborate with healthcare professionals to deliver integrated care.
- Advocate for clients and link them to vital community resources.
Requirements
Qualifications :
- Master’s degree in Social Work
- Licensed Clinical Social Worker (LCSW) credential
- Experience with older adults or specific client populations preferred
- Excellent communication and clinical skills
- Ability to work autonomously and as part of a supportive team
