SHIFT : Business hours are typically - but due to the need to documentation is required to be submitted within turn around times it is important to note that more than hours would be required at time to complete.
Visits can run after pm due to the population of children being assessed and would need to work with both the member / parents scheduled.

Walk me through the day to day responsibilities of this the role and a description of the project :
Care Manager would be responsible to complete assessments per week with our members in their home. Our position is with the Assessment Team and we are the ones to complete initial assessments for new members and we also complete reassessments for Level members (low acuity).
Our team also completes Medically Dependent Children’s Program Interest List assessments.
Describe the performance expectations / metrics for this individual and their team :
- Attend onboarding training
- Complete - weeks of shadowing with a preceptor / tenured staff
- Independently complete visits / assessments within a weeks’ time frame and all applicable documentation required by contract.
Assessment can take - hours Documentation can take hours
- Submit all required documentation within the BD time frame unless otherwise indicated for BD
- Complete all trainings on time
- Attend all team meetings
Job Duties :
Performs care management duties to assess and coordinate all aspects of medical and supporting services across the continuum of care for complex / high acuity populations with primary medical / physical health needs to promote quality, cost effective care.
Develops a personalized care plan / service plan for long-term care members, addresses issues, and educates members and their families / caregivers on services and benefit options available to receive appropriate high-quality care.
Education / Experience :
Requires Graduate from an Accredited School or Nursing or a Bachelor's degree and years of related experience
Bachelor's degree in Nursing preferred
License / Certification :
RN - Registered Nurse - State Licensure and / or Compact State Licensure required or
NP - Nurse Practitioner - Current State's Nurse Licensure required
