WHAT DOES YOUR DAY ENTAIL?
As an LPN in our Care Management team, you will have the thrilling opportunity to assist our team in providing exceptional care for our patients.
Your core responsibility is to support the Medical Provider (MD and NP), ensuring clients are prepared for appointments.
You'll manage integrated care functions, connect with external providers, coordinate scheduling, facilitate referrals, and deliver patient and family support.
Additionally, you'll perform crucial medical tasks like administering treatments, recording vital signs, and managing lab tests, all while maintaining meticulous documentation and a culturally sensitive approach.
You'll also handle medication-related calls, prior approvals, abnormal lab results, and inventory management, showcasing your dedication to exceptional patient care through versatile responsibilities.
DO YOU HAVE WHAT IT TAKES?
- Completion of a state-approved certified nursing program
- Successful passing of a competency program that meets state regulations
- Unrestricted certification as an LPN
- 6+ months of experience as an LPN in a healthcare / psychiatric setting
- Licensed Practical Nurse with a Tennessee license
- Experience in an office setting
