Licensed Care Management Nurse / Social Worker

A company is looking for a Care Management Nurse or Social Worker.

Key Responsibilities

Conduct assessments and develop care plans tailored to individual member needs

Implement and coordinate case management activities to achieve established goals

Monitor and evaluate the effectiveness of care plans and recommend improvements

Required Qualifications

Associate or Bachelor's Degree in Nursing or a related field

3 years of relevant case management or clinical experience

Current licensure or certification in a health or human services discipline

Ability to conduct assessments independently within the scope of practice

Experience in clinical care, including direct patient interaction
