This role involves collaborating with healthcare teams, assessing patient requirements, diagnosing conditions, and devising treatment strategies.
Responsibilities : Plans and organizes patient care according to protocols and pathways.Collaborates with the healthcare team to develop and implement treatment plans.
Conducts health assessments and orders necessary tests.Prescribes medications and medical equipment as per practice protocols.
Evaluates treatment effectiveness and modifies plans as needed.Promotes professional growth and serves as a clinical resource.
Assists Clinical Access Center staff when necessary.Completes additional tasks as directed by supervisorQualifications : License : Current registration as Nurse Practitioner in New York StateEducation : Master's degree in nursingCertification as an Adult or Family Nurse Practitioner
