Summary
Provide direct patient care under the direction and supervision of a registered nurse in a clinical setting.
Qualification Requirements
To perform this job successfully, an individual must be able to participate in the assessment of patients; use critical thinking skills in response to events, observations, experiences, verbal and written communication, problem solving and decision making.
Minimum Qualifications
- Current license to practice as an LPN in the state of North Carolina.
- Minimum of 2 years of clinical experience
- Current BLS or ACLS certification
- 2 years of Nephrology experience, preferred.
- Passage of a criminal background check, OIG exclusion and drug screenings
- Valid driver’s license and reliable transportation
- Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
Knowledge, Skills, and Abilitie s
- Computer skills using Microsoft (MS) Excel and SharePoint, MS Word, MS Outlook, MS Teams, and various data management platforms.
- Comprehensive knowledge of the field’s concepts and principles
- Exceptional people skills to provide excellent patient care.
- Skill in organizing resources and performing repetitive tasks.
- Ability to function well while involved in multiple task assignments.
- Knowledge of ethics of confidentiality and the ability to maintain confidentiality of sensitive information.
- Follows principles of medical ethics for medical assistants.

Essential Functions 100%
Gathers necessary information to determine the needs prior to the doctor’s exam. Obtain patient vital signs. Blood pressure, temperature, and pulse (radial or apical).
Obtain patient weight and record information in the EMR.
- Review allergies, prescription medications, and over the counter medications 25%
- Perform laboratory work with scope of practice.
- Check HGB of patient via fingerstick after training and certification by the Physician(s), Nurse Manager, Nurse Practitioner(s), Physician Assistant(s), or other Qualified Personnel as required per protocol and document in the patient’s electronic medical record (EMR).
- Read stool cards as required, and documents results in the stool card template in the EMR. 25%
- Participates in Chronic Care Management 10%
- Gives instructions to patients as instructed / ordered by doctor. Makes specialty referrals as ordered by doctor and documents medical information in the electronic medical record 25%
- Maintain accurate medical records by documenting patient communication, changes in intervention per medical provider, referral calls, pharmacy calls, and triage calls in the EMR 15%
Physical Demands
- Hearing, visual acuity, depth perception, balancing, handling, and talking.
- Reaching, grabbing, holding fine motor skills.
- Extended periods in a stationary or standing position
- Repetitive motion such as entering data into computer-based programs.
- Lift or move up to twenty-five pounds. Employee is expected to use appropriate ergonomics and tolls such as hand carts for heavier loads.
Work Environment
Work is performed in a typical interior / office work environment. The noise level is usually moderate. While performing the duties of this job, the employee is regularly required to stand, talk, or hear.
