Care Management Coordinator
Job Description
We are seeking a highly organized and detail-oriented Care Management Coordinator to join our team.
As a Care Management Coordinator, you will be responsible for coordinating and facilitating care management services for our clients.
You will play a critical role in ensuring the smooth and efficient delivery of care management solutions.
Responsibilities
- Coordinate and manage care management services for clients
- Collaborate with healthcare professionals to develop effective care plans
- Monitor and evaluate the progress of care management programs
- Ensure timely and accurate documentation of client information and care plans
- Provide guidance and support to clients and their families
- Maintain strong relationships with healthcare providers and community resources
Requirements
- Proven experience in care management or a related field
- Excellent organizational and time management skills
- Strong attention to detail
- Exceptional communication and interpersonal abilities
- Ability to work effectively in a team environment
- Proficiency in medical terminology and documentation
- Bachelor's degree in healthcare administration or a relevant field
