Position Overview : We are seeking a dedicated Nurse Practitioner to join our care team. You will play a pivotal role in providing comprehensive healthcare services to patients throughout their healthcare journey.

Key Responsibilities :
- In-Home Assessments (IHA) : Conduct comprehensive in-home assessments for patients, including medical history reviews, physical examinations, and medication management.
- Patient Assessment : Conduct thorough assessments of patients' health status, including physical examinations, medical histories, and risk assessments.
- Diagnosis and Treatment : Formulate accurate diagnoses and develop individualized treatment plans based on evidence-based practices and guidelines.
- Chronic Disease Management : Manage and monitor chronic health conditions, such as diabetes, hypertension, and cardiovascular diseases, ensuring optimal disease control and patient education.
- Medication Management : Prescribe medications and adjust dosages as necessary, while educating patients on proper medication administration and potential side effects.
- Preventive Care : Promote preventive healthcare measures, including immunizations, screenings, and lifestyle modifications, to improve patient health and prevent future illnesses.
- Patient Education : Provide patient education on disease management, healthy lifestyles, and self-care practices to empower patients to take an active role in their health.
- Collaborative Care : Collaborate closely with physicians, nurses, and other healthcare professionals to ensure coordinated care and seamless transitions for patients.
- Primary Care Provider of Record : Serve as patients’ primary care provider.
- Documentation : Maintain accurate and up-to-date electronic health records (EHR) to track patient progress, treatment plans, and interventions.
- Bedside Manner : Exhibit exceptional bedside manner, fostering trust and rapport with patients and their families through genuine care and empathy.
- Follow-Up : Schedule and conduct regular follow-up appointments with patients to assess treatment effectiveness and make necessary adjustments.
- Quality Assurance : Participate in quality improvement initiatives and adhere to healthcare regulations and standards to maintain the highest level of care quality.
- After-Hours On-Call : Participate in after-hours on-call and conduct longitudinal care and IHA visits as needed.
- Travel : Drive to client and / or patient sites within a designated area.
- Flexibility : Other duties as assigned based on business needs.
