• Key Responsibilities :
• Conduct intake visits to assess client needs and develop personalized care plans.
• Perform periodic care plan visits to ensure ongoing client support and satisfaction.
• Facilitate medication setup visits to assist clients in managing their medications safely.
• Train and supervise aides in providing elder care and assistance with Activities of Daily Living (ADLs).
• Maintain clear communication with the care team to ensure coordinated care for clients.
• Conduct client satisfaction visits to gather feedback and improve service quality.
• Manage daily administrative duties, including visit reviews and approvals.
