Responsibilities :
- Implement physicians’ prescribed plan of care
- Conduct patient assessments and coordinate care
- Administer prescribed medication, treatments, and therapies
- Document care provided in a timely manner
- Promote health, teach, and train family members
- Accept and write verbal orders from physicians, obtaining co-signature from Case Manager
- Follow up with physician, client, family members, staff, and Case Manager on client concerns or changes in condition
- Perform other appropriate duties as assigned
Qualifications :
- Active, unencumbered license (RN / LPN / LVN) in the state of practice
- Current CPR certification from American Red Cross, American Heart Association, or BLS
- Current TB skin test (within the last 12 months)
- Six months of hands-on nursing experience preferred but not required
- Reliable transportation
