Eliot’s Behavioral Health Division provides an integrated and comprehensive network of services to adults with psychiatric disabilities that are evidence based, provide rehabilitation & hope to those in services.
PACT staff will coordinate and provide comprehensive recovery and rehabilitative services to consumers participating in PACT.
Interventions include a range of therapeutic & behavioral services, including crisis intervention, for individuals with both severe mental illnesses and / or substance abuse disorders.
Services are predominantly community based, including at consumers' homes, medical facilities, & other locations.
Responsibilities :
- Provide case management services as part of a multidisciplinary team
- Develop treatment plans with consumers.
- Act as a primary case manager to consumers in accordance with Eliot’s policies and PACT standards.
- Participate in completing assessments as assigned by the Team Leader or Lead Clinician in a timely manner.
- Assess client's functions, strengths and needs.
- Develop therapeutic connections with each client and provide clinical interventions.
- Provide on-call on a rotating basis and rotating evening shifts.
- Provide problem-solving supports and interventions to clients
Schedule : Monday - Friday 8am to 4pm with standing 12pm-8pm shift. Includes rotating weekends and on-call.
