What You'll Do

• Oversee day-to-day operations of the community-based services team, ensuring safe, high-quality care for participants.
• Provide leadership, coaching, and supervision to nurses, aides, and support staff — including hiring, training, scheduling, and performance management.
• Monitor compliance with regulations and internal policies, while maintaining positive staff morale and a collaborative work culture.
• Conduct initial and annual assessments of participants to identify needs and coordinate individualized care plans.
• Collaborate with interdisciplinary team members to ensure seamless care delivery and optimal outcomes.
