We are constantly growing to meet the needs of our large network of hospitals, physicians, and community partners. Our home health structure is set up to provide Home Health Nurses multiple avenues for support that aren't available at other agencies a field preceptor, an IT specialist, a dedicated scheduler, a nurse preceptor during orientation (available every day to help with documentation questions and challenges), an in office team of QI specialists, telehealth resources, clinical manager and in office triage support.

As a Home Health Registered Nurse, you will join the Banner Home Health Clinical Team providing exceptional, expert care in your community.
You will enjoy an autonomous practice focusing on medical intervention within the patient’s actual home environment in partnership with a team of skilled and experienced home care case managers.
The Home Health space allows for the time to create one on one relationships with your patients and their caregivers and have some flexibility over your schedule that is not possible in the hospital setting.

We offer mileage reimbursement and will provide you a company cell phone and a tablet for documentation in our electronic medical record system (Home Care Home Base).
Must have recent Home Health or Hospice experience, one year minimum.
Banner Staffing Services (BSS) also offers Registry / Per Diem opportunities within Banner Health. Registry / Per Diem positions are utilized as needed within our facilities.

BSS Registry positions do not have guaranteed hours and no medical benefits package is offered . BSS requires Completion of post-offer Occupational Health physical assessment, drug screen and background check (includes;
employment, criminal and education).
POSITION SUMMARY
This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and in accordance with standards of professional nursing practice utilizing the framework for professional nursing practice and development.
This position is accountable for the quality of nursing services delivered by self or others who are under their direction.
This position utilizes specialized knowledge, judgment, and nursing skills necessary to assess data and plan, provide and evaluate care appropriate to the physical and developmental age of assigned patients.
CORE FUNCTIONS
1. Assesses patient physical, psychological, social, spiritual, educational, developmental, cultural and discharge planning needs.
Reviews patient history and physical with patient / family and assures completion within appropriate timeframe. Reviews available information obtained by other health care team members.
Reviews diagnostics and laboratory data and reports abnormal results to the physician(s) and other appropriate caregivers.
Completes assessment and reassessments according to patient need and as outlined in policy.
2. Formulates a plan of care, including the discharge plan, utilizing assessment data and patient, family and health team input.
Initiates a plan of care based on patient-specific needs, assessment data and the medical plan of care. Goals for patient are realistic, measurable and developed in conjunction with the patient / family.
Considers the physical, cultural, psychosocial, spiritual, age specific and educational needs of the patient in the plan of are.
