Home Health Registered Nurse

We are offering a $20,000 sign-on bonus for this position.
Responsibilities :
- Provide admission, case management, and follow-up skilled nursing visits for home health patients.
- Administer ongoing care and case management, coordinating with healthcare personnel and reporting patient progress.
- Confer with physician to develop initial plan of treatment based on patient assessment and physician's orders.
- Coordinate / oversee / supervise the work of other healthcare providers and provide written instructions / care plans.
- Prepare appropriate medical documentation on all patients and confer with other healthcare disciplines.
Requirements :
- Diploma, Associate or Bachelor Degree in Nursing
- Minimum of one year nursing experience preferred
- Current and unrestricted Registered Nurse licensure
- Strong organizational and communication skills
- Valid driver's license, auto insurance and reliable transportation
Scheduled Weekly Hours : 40
Pay Range : $80,000 - $110,000 per year
