T heCase Manager positionwill :
- Support the physician primary medical homes and interdisciplinaryteams.
- Facilitatepatient care with the underlying objective of enhancing the qualityof clinical outcomes and patient satisfaction while managing thecost of care and providing timely and accurate information topayors.
- integratesand coordinates resource utilization management care facilitationand discharge planning functions.
- Help to drive changeby identifying areas where performance improvement is needed (e.g.day to day workflow education process improvements patientsatisfaction).
- coordinate a widerange of selfmanagement support and provides information to updateand maintain relevant disease registryactivity.
- Accountablefor a designated patient caseload and plans effectively in order tomeet patient needs across the continuum provide family supportmanage the length of stay and promote efficient utilization ofresources.
MinimumQualifications
Education : Graduate of an accredited school of professional nursing required;Bachelors of Nursing preferred or graduate of an accredited Masterof Social Workprogram.
Licenses / Certifications :
Currentand valid license to practice as a Registered Nurse in the state ofTexas or
Current and valid license as a MasterSocial Worker (LMSW) in the state of Texas required LCSWpreferred.
Certification in Case Managementrequired within two (2) years of hire into the Case Managerposition.
