About The Position
Job Title : Social Worker Job Specialty : Home Care Social Worker Job Duration : 13 weeks Shift : 08 : 00 AM - 04 : 30 PM Guaranteed Hours : Not specified Experience : Minimum 1 year of experience in Home Care and Social Work License : Current licensure in the state of Massachusetts Certifications : None specified Must-Have : - Reliable, independent means of transportation - Willingness to work weekends, holidays, and on-call as required by department rotations (once every 6 weeks) Job Description : Provide and coordinate social services for patients ranging from birth to over 100 years in their homes.
- Advocate for patients dealing with psychological, health, family, and financial challenges.- Assess the mental and emotional stability of patients by understanding their social and financial condition, along with their family dynamics.
- Coordinate and collaborate with healthcare workers to provide optimal health-care services to the patients.- Utilize thorough and timely electronic documentation and interdisciplinary communications as per agency policies and procedures.
- Work to meet and uphold standards of practice in home care settings. Unit Description : The open position is for a Home Care Social Worker covering the Blue Hills Team (Dedham, Hyde Park, Roslindale, West Roxbury) and the Paul Revere Team (Dorchester, Jamaica Plain, Mission Hill, Roxbury).
Applicants should be comfortable with travel in these areas.
