Job Summary
In this role, you will primarily be creating and managing a plan of care for individual patients. You are a Registered Nurse (RN) who operates with great attention to detail and a big heart.
You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient's best interest.
To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong penchant for solving problems.
We are seeking a Registered Nurse who lives in the Brooklyn Park community to join our team. We're looking to hire a full time or part time nurse who is competent to perform the following task including but not limited to :
Responsibilities
- Performs focused nursing assessment of Residents to determine appropriate interventions to maintain or improve the Resident’s health status.
- Provides interventions for Residents with episodes of potentially life-threatening instability.
- Provides nursing visits and direct care to Residents in the resident's home or assisted living as directed by client Care Plans.
- Sets up medications for Residents according to the medication management program, as indicated.
- Coordinates medical appointments for Residents, including follow-up with any new orders or changes after hospital visit with direction from Nursing case manager as needed
- Accepts and implements prescriber orders with direction as needed
- Responds to phone calls from other providers related to Residents with RN direction as needed
- Documents, as indicated, in the clinical record
- Communicates with families, resident representatives, case managers and other health care providers regarding the Residents as needed
- Is aware of and uses good safety practices, including but not limited to :
- Body mechanics
- Falls prevention
- Reporting of defective equipment or hazards
- Wiping spills or wet floors immediately
- Using Standard Precautions and appropriate infection control measures
- Knowledge of emergency procedures
- Reporting incidents / accidents to the Clinical Nurse Supervisor or Director on the day of occurrence
- Participates in care conferences with other members of the health care team
- Maintains absolute confidentiality of all information pertaining to Residents, families, and employees
- Promotes a safe and comfortable environment for the Residents
- Demonstrates excellent written and verbal communication skills
Qualifications
- Currently licensed Registered Nurse (RN) in state of employment
- Previous experience as a Registered Nurse is preferred
- Excellent interpersonal and client care skills
- Valid driver’s license
- First aid / CPR certification required
