Job Summary

The Speciaty Licensed Practical Nurse provides general nursing careto patients, families and caregivers in the Medical Grouppractices.

The Licensed Practical Nurse assists and advocates forthe patient and families health in support of the patient centeredapproach and delivery model of care, and is an active care teammember.

Responsible for collaborating with physicians / providers ,registered professional nurses and multidisciplinary care teammembers.

A Licensed Practical Nurse can perform tasks andresponsibilities within the framework of case finding, healthcoaching, and supportive care under the direction of the registeredprofessional nurse or licensed physician or licensed health careprovider.

Nursing shall perform in accordance with the standardsoutlined for Licensed Practical Nurse in the NYS Education Law, NYSOffice of the Professions, and by the State Nurse Practice Act.

ALicensed Practical Nurse will follow the mission, vision, I-CAREvalues, policies and procedures of Mohawk Valley HealthSystem.

• Gather patientmeasurements and collect patient data
• Recordand report patient findings
• Prepare patient forpatient examination and treatment
• Assistswithexamination and treatments
• Perform tasks withinthe framework of supportive care, under the clinical direction oflicensed health care provider or registered professionalnurse
• Support self-care , self- management andthe established plan of care.
• Utilize standardprecautions and practice at all times
• Maintainequipment, supplies, inventory, and a safe and clean workenvironment
• Maintain technical knowledge byattending educational workshops, mandatory in-services; reviewingpublications and participating in department committee meetings orwork groups
• Engage in quality improvementinitiatives including continuing education, participation insystem-wide and department specificteams
• Maintain a cooperative relationship amongthe healthcare team by effectively communicatinginformation
• Respond to patient requests,communicates effectively to the licensed health care provider whocan fulfill the patient request

Education / Experience Requirements

Required :

Graduate of anaccredited school of licensed practical nursingprogram

Preferred :

Bachelors or Masters of Science in Nursing fromaccredited nursing program

Licensure / Certification Requirements

Required :

• Current New YorkState licensure as a Licensed PracticalNurse
• Basic Life Support Certification - Mustreceive BLS certification within 6 weeks of hiredate
