POSITIONSUMMARY : The Clinician II will work collaborativelywith physicians, staff and other health care professionals toprovide a smooth health care continuum for all patients within theclinic setting.
The nurse is an integral member of the clinic careteam who works to ensure safety, best practice and high qualitystandards of care are maintained across the continuum.
TheClinician II is responsible for coordinating a variety of patientwellness and chronic illness activities for the clinic’s entirepatient population.
Success will be measured by the results of theoutcome performance measures of the population of patients in theclinic. (e.
g. Patient Satisfaction, ACO, HCC, clinic qualitydashboards). The Clinician II acts in a clinical leadership roleutilizing concepts of critical thinking and creative problemsolving to ensure positive outcomes in the delivery of clinicalcare.
Requirements
MINIMUMQUALIFICATIONS :
EDUCATION : Graduate of anaccredited program for Registered Nursing. RN Required. BSNpreferred.
CERTIFICATION / LICNESES : Licensed by State of New Mexico as a RegisteredNurse. BLS Certification issued through American Heart Associationrequired, or within 2 weeks of hire date.
SKILLS :
- Knowledge of and practical use of good business English,spelling, arithmetic, practices
- Ability tocommunicate effectively using written and verbal skills
- Demonstrates effective teaching techniques, applyingadult learning principles
- Demonstrates theability to coordinate and implement appropriate educationalmaterials for patients and their support systems
EXPERIENCE : Oneyear experience working as a nurse in a clinical settingpreferred.
NATURE OFSUPERVISION :
Responsible to : CSVCGDyad
In conjunction with clinic leadership anddyads :
- Supervises daily activitiesof medical assistants.
- In conjunction withClinic leadership ensures timely orientation and competenciescompletion for all clinical support staff.
- Ensures provision of uninterrupted clinical care.
- Provides feedback to manager for annualperformance evaluations of clinical staff.

PHYSICAL REQUIREMENTS : Requires full range of body motion includinghandling and lifting patients, manual and finger dexterity andeye-hand coordination.
Requires standing and walking forextensive periods of time. Occasionally lifts and carriesitems weighting up to 50 pounds.
Requires working understressful conditions or working irregular hours. Frequentexposure to bodily fluids, communicable diseases, toxic substances,ionizing radiation, medicinal preparations and other conditionscommon to a clinic environment.
