Job Summary

The Medical Coordinator (MC) is responsible for assisting on ensuring the quality of medical services, treatment, and / or interventions provided to Unaccompanied Children (UC) in the program in collaboration with the contracted physicians. Medical Coordinator is the key monitor of patients' health through observing and assessing their records, symptoms, and reactions to treatment and care. They often have extensive interaction with the UC as well, guiding and instructing them in aftercare measures. The MC assist on the delivery of medical care with the emphasis on disease prevention and health maintenance while ensuring compliance with State licensing, Office of Refugee Resettlement (ORR), and all policies and procedures implemented by DSS.

Essential Functions :

  • Coordinate appropriate health services, including urgent, emergent, and preventative health services, for children in care. This includes maintaining complete, confidential medical records for children, ensuring appropriate communication of health needs to case managers and clinicians, scheduling health care appointments.
  • Coordinating transportation to health care appointments, ensuring qualified interpretation and translation services are in place, and serving as a liaison for community health care providers.
  • Provide support in coordinating medical, dental, and mental health services to meet the needs of the children and ensure that all new arrivals receive their initial medical assessments within 48 hours as required by ORR.
  • Coordinate the daily administrative activities, including scheduling and client medical record maintenance. Prepare client report transcription from physician record. Assure charts and / or files are recorded and purged as required.
  • Prepare treatment rooms for medical examinations and assist practitioners with medical procedures and other patient care.
  • Implement physicians’ orders, create nursing plan of care, administer medications, perform treatments, procedures and special tests, and document treatment as directed by physicians and required by company policy and local / state / federal rules and regulations. Ensure compliance with all health care regulations, including HIPAA and OSHA.
  • Ensure all infection control, OSHA, CDC, and State licensing guidelines are followed and maintained in all medical departments.
  • Counsel youth concerning their medical status, disease, treatment, and prevention of disease and other health related topics.
  • Assist on providing quality assurance for the medical department to include the following : Oral, written, and statistical reports, auditing of electronic medical records, maintaining all medical department physical records and electronic systems and all medical documents completed and provided by the medical department.
  • Responsible for reporting requirements under the program for the Health Department. Maintain and implement any recommendations given by the Public Health Departments, CDC, supervisor, Occupational health and Safety specialist and ORR.
  • Perform and coordinate clinical care as follows : maintain medical supplies and prescribed medications, follows standing orders, State licensing, and CFRC policies and procedures for providing over the counter medications to minors, prepares youth for examinations, and any other clinical tasks as needed.
  • Coordinate youths’ vaccination and maintain vaccination records as ordered by health care provider or per medical department protocols.
  • Provide primary and emergency care for injuries and illnesses.
  • Support development and implementation of service planning to ensure that all youth have access to diverse and meaningful program offerings.
  • Attend organizational required training and facilitate trainings as needed.
  • Maintain a safe, clean, and hazard-free work area.
  • Other tasks and responsibilities as assigned.

Qualifications and Requirements

  • Licensed practical or vocational nurse (LPN / LVN) or higher health-related qualification (e.g., licensed registered nurse (RN) with a bachelor’s degree)
  • Minimum of 1 year of employment in public health or a healthcare setting with demonstrated experience in care coordination, health administration, or direct clinical care.
  • Must be bilingual (English / Spanish) - speak, read and write
  • Working Conditions / Physical Effort

    Work is performed mostly in an office setting. Some overnight travel, public speaking, evening and weekend work are requirements of the position. Regular computer works also required. While performing the duties of this job, the employee is required to stand; walk; sit; finger, handle, or feel objects, or controls; reach; climb stairs; balance; stoop, kneel, or crouch; talk and hear. The employee must occasionally lift and / or move up to 20 pounds. Are exposed to diseases or infections on a daily basis. Always wear masks or rubber gloves to protect themselves and their patients from disease. Usually work indoors. Work very near patients and doctors. They come into physical contact with patients throughout the day. Usually wear a uniform or lab jacket. Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
