Job Description

Tallahassee Memorial Healthcare (TMH) is a private, nonprofit community-based healthcare system that provides care to a 22-county region in North Florida and South Georgia. We are a career destination with over 6,000 colleagues who reflect the diversity of our community.

  • Level II Trauma Center
  • Primary Stroke Center
  • Level III Neonatal Intensive Care

  • The most advanced cancer, heart and vascular, orthopedic & surgery programs in the Panhandle.

Our system also includes a psychiatric hospital, multiple specialty care centers, six residency programs and more than 50 affiliated physician practices.

Qualifications

Required Education :

  • Doctor of Physical Therapy (DPT) degree (or equivalent) from a Commission on Accreditation in Physical Therapy Education (CAPTE)-accredited educational program. Master’s degree (or equivalent) in physical therapy from a CAPTE-accredited educational program conferred prior to 2019 and bachelor’s degree (or equivalent) in physical therapy from a CAPTE accredited educational program conferred prior to 2000 may be accepted

    • Required Experience :

  • 1 year

    • Required Certification / License / Registry :

  • Licensed as a physical therapist in accordance with Florida Statutes (FS), Title XXXII (Regulation of Professions and Occupations), Chapter 486 (Physical Therapy Practice). Temporary permit may be accepted. Current Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification from an accredited instructor from the American Heart Association (AHA). Current Florida class “E” driver license.

    • Job requires a valid state driver license. Required to be at least twenty-one (21) years of age and with a minimum of two (2) years of licensed driving experience. Required to provide a seven (7)-year driving record from the Department of Motor Vehicles (DMV) prior to employment. Will have received no more than two (2) minor violations in the past three (3) years and zero (0) major violations in the past five (5) years.

    NOTE : Use of personal vehicle(s) required. Vehicle owner must maintain adequate insurance coverage for personal vehicle(s) used in conjunction with duties. Tallahassee Memorial HealthCare (TMH) will reimburse the business use of personal vehicle(s) in accordance with procedures defined by the Internal Revenue Service (IRS).

    Responsibilities

    Job Summary :

  • Delivers safe and competent physical therapy as ordered by a physician.
  • Plans, organizes and implements specific treatment programs for individual patients according to the principles and practices of physical therapy.
  • Collaborates with interdisciplinary treatment team.
  • Supervises work of support personnel, physical therapy assistants and home health aides.

    • Reports : Administrator / Home Health Care, Clinical Supervisor, or Clinical Manager

    Supervises : Physical Therapy Assistant (JC : 041702)
