Inpatient Case and Utilization Management Registered Nurse, Care Coordination (Full Time)

Supports the physician and interdisciplinary team in facilitating patient care, with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction while managing the cost of care and providing timely and accurate information to the interdisciplinary team.

The role requires both utilization management and case management and will be able to work in each division of care coordination department as needed.

This role will provide patient care facilitation advocacy and executes discharge planning functions related to medical needs.

The job is to be accountable for a designated patient caseload and plan effectively to meet patient needs, manage the length of stay, and promote efficient utilization of resources.

Must be able to perform a comprehensive assessment of patient, coordinate post discharge care of patient, facilitate precertification and payor authorization processes of medical patient and facilitate care across the continuum.

Demonstrates the ability to deal with pressure to meet deadlines, to be accurate, and to handle constantly changing situations.

Demonstrates the ability to deal with a variety of people, deal with stressful situations, and handle conflict. Demonstrates the ability to coordinate and communicate with physicians regarding the progression of care, the need for clinical and family care conference in collaboration with the care team and the coordination for post-hospitalized needs of the patient.

Will perform all job duties or job tasks as assigned. Will follow and adhere to all requirements, regulations and procedures of any licensing board or agency.

Must comply with all Benefis Health System's organization policies and procedures.

Education / License / Experience Requirements :

  • Current state registered nurse license required
  • Bachelor's or Associates degree in nursing or related field required
  • Case Management (CCM) or American Case Management Certification (ACM) preferred
  • Professional certification as a Case Manager within three years of employment required
  • Three to five years' experience direct patient care and / or case management required
  • Financial and discharge planning experience is required
