Role Summary :
As a Care Manager LCSW, you will play a critical role in assessing and coordinating care for a diverse patient population.
Collaborating with a multidisciplinary team, you will work closely with patients and their families to ensure the delivery of safe, high-quality, and effective care.
This position offers the opportunity to contribute to a culture of patient-focused, whole-person care within our health center.
Key Responsibilities :
Care Coordination : Collaborate with patients, families, and a multidisciplinary team to manage and coordinate care effectively.
Assessment : Utilize your skills to assess patient needs, considering social determinants of health, mental health, substance use, and other psychosocial issues.
Case Management : Facilitate communication and care coordination throughout the continuum of care, ensuring effective transitional care management.
Patient Advocacy : Support patient self-determination and address barriers to coping and access to resources.
Other duties as assigned.
Required Qualifications :
Master’s Degree in Social Work from an accredited program.
California Clinical Social Worker License upon hire.
Two years of experience in a healthcare-related field
Two years of experience in Acute Care Management
