At Valley Regional Hospital, patients are the center of our coordinated care system and right now we have an opening for a Registered Nurse / Care Coordinator to join our team in Orthopedics.
The primary responsibility of the RN / Care Coordinator is : responsible for coordinating, providing and documenting care for patients before and after surgery.
Utilizing providers' orders and professional skills, the RN Care Coordinator will follow up with patients after discharge.
Chart updates from discharge to reflect the discharge plan, medications and new diagnosis in compliance with federal and state regulations, reimbursement guidelines as well as the policies and procedures of Valley Regional Hospital.
An ideal candidate would have the following qualifications :
- Bachelor's or equivalent related experience recommended.
- Excellent customer service and communication skills
- Outstanding ability to exercise discretion and confidentiality with sensitive information
- Excellent organizational skills with the ability to work independently through interruptions
- Ability to prioritize and quickly change tasks
