The University of Iowa Hospitals and Clinics Department of Nursing is seeking a Discharge Nurse Clinician to perform specific tasks to assist members of the Nursing team in providing patient education and discharge planning under the supervision of the Director of Pediatric Care Coordination Division.
Responsibilities will include, but are not limited to :
- Plans, monitors, and documents patient care across the continuum from inpatient and outpatient services
- Educates patient and family about aspects of treatment plan.
- Serve as a patient advocate as well as assist families to develop advocacy skills.
- Apply expert skills to facilitate assessment of patient needs throughout episode of care.
- Complete community referrals upon discharge (AEA, Early Access, school systems, etc.)
- Observes for patterns of breakdown in plan of care and facilitates correction of problems.
- Collaborate and communicate with health care providers from external agencies to assist the facilitation of a smooth transfer to and from the hospital setting.
- Identify performance opportunities and assist with implementation of performance improvement plans for specific programs or specific populations.
- Collaborates with nursing / staff to monitor continuity of care for the patient.
- Coordinate and / or assist in the planning process for Iowa's Medical Home and Community-Based Waiver programs.
- Provide follow-up phone calls to families of children with complex special healthcare needs who have been enrolled in the Continuity of Care Program.
- Coordinates and documents communication with the patient post-discharge to answer questions and follow-up on any concerns.
- Be a liaison to families, insurance companies and community providers and assist in securing the in-home community-based services needed for the child to be discharged from the University of Iowa Stead Family Children's Hospital.
- Assist children with special healthcare needs and related services, systems and program development activities as requested.
Percent of Time : 50%
Schedule : 20 hours per week, scheduled between Monday-Friday, no weekends, no holidays

Required Qualifications :
- A Baccalaureate degree in Nursing
- Minimum of 2 years recent NICU nursing experience in children's hospital
- Current license to practice nursing in Iowa
- Demonstrated expertise in specific clinical area of practice
- Demonstrates skill and efficiency in coordination of care and discharge planning for pediatric patients with a complex health care needs (involving multiple departments and external agencies)
- Demonstrates excellent interpersonal and communicatiion skills with patients, families, interdisciplinary team members and community providers / agencies
- Demonstrates effective teaching skills for staff and patient / family education. Ability to develop creative staff, patient and community education / training programs.
- Demonstrates willingness to flex work schedule to accommodate patient / family needs and participation in community outreach
- Experience in Quality Management
- Demonstrates ability to meet essential / marginal job funtion analysis
- Proficiency in computer applications including Microsoft Excel, Word and PowerPoint
- Excellent written and verbal communication skills
- Demonstrated excellent organizational skills, critical thinking, and problem-solving skills
- Demonstrated expertise with pediatric patient / family discharge planning process
- Experience working with an interdisciplinary team
Desired Qualifications :
- Previous case management experience is desired
- Certification in case management / pediatric nursing is desired
- Previous experience with EPIC is desired
