MedCadre is seeking a LPN / LVN Home Health for a local contract job in Lacey, Washington.

Position Title :  Licensed Practical Nurse- Home Health Nurse

Type : 13 Weeks Contract

Hourly Pay Rate :  $44- $50 / Hour

Location :  Lacey, WA 98503

Shift :  Day 5x8-Hour (08 : 00 - 16 : 30)

Summary

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team as a Home Health Nurse. In this role, you will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being.

Duties

  • Conduct comprehensive patient assessments and develop individualized care plans.
  • Administer medications and treatments as prescribed, including IV infusion and injections.
  • Monitor vital signs and document patient progress accurately in electronic health records (EHR).
  • Provide wound care, catheter care, and assistance with activities of daily living (ADLs).
  • Ratios : 1 : 1 (
  • May visit up to 6 patients per day; 30 productivity points standard)

    • Qualifications

  • Require 1+ year of recent Skilled Home Health Experience (at least 1 year within the last 3 years)
  • Must have WA license, or compact license
  • Familiarity with EMR systems such as Epic, Cerner, or eClinicalWorks is an asset.
