RN Case Manager
The RN Case Manager will be responsible for providing comprehensive case management services to clients in the clinic and community.
The successful candidate will be a compassionate, organized professional with excellent communication and problem-solving skills.
Responsibilities :
Provide case management services
Develop and implement individualized care plans for clients
Monitor and evaluate client's progress
Provide crisis intervention and support
Collaborate with other healthcare professionals to ensure quality care
Maintain accurate and up-to-date client records
Home visitation as needed
Other duties as assigned.
Qualifications :
Registered Nurse (RN) license in the state of California
Minimum of 3 years of experience in case management
1 year of working with the geriatric population
Baccalaureate degree in nursing.
Bilingual preferred
