Per-Diem / Full-Time Position
We are seeking an Occupational Therapist who is passionate about delivering top-notch Occupational Therapy services.
KEY RESPONSIBILITIES
Conducts assessments, diagnostic tests, skilled treatments, and continuous evaluation of clients receiving services based on an approved medical care plan.
a. Completes assessments promptly and adhering to Agency protocols. b. Develops treatment programs and establishes goals for enhancing functionality.
c. Communicates assessment findings and goals to assist in formulating the client's care plan and obtains necessary physician orders.
d. Documents all findings, plans, interventions, and client progress using designated forms following Agency guidelines.
e. Shares plans and updates with the physician, nursing Case Manager, and other Agency staff through the care plan, progress notes, and active participation in care conferences.
Guides and trains clients in therapeutic self-care and creative activities aimed at improving independence, physical, and cognitive functioning.
Advises, consults, and educates clients, family members, and other Agency staff on appropriate treatment procedures and supportive activities.
Designs and / or orders suitable adaptive / assistive devices and splints to optimize functionality. Provides client-specific program instruction to clients and their families.
Ensures compliance with Agency policies, procedures, and professional and community standards. a. Renders services in accordance with applicable state, federal, and JCAHO standards.
b. Attends essential meetings and educational programs. c. Takes responsibility for personal and professional growth, identifying learning / developmental needs in consultation with the supervisor.
Attends care conferences, initiates interdisciplinary referrals, and collaborates in care coordination as necessary. Serves on the Clinical Record Review Committee.
Performs additional duties and responsibilities as deemed necessary.
Qualifications :
Graduate of an accredited occupational therapy curriculum, jointly accredited by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association;
or eligible to take the National Registration Examination of the American Occupational Therapy Association.
- Holds current registration or license as mandated by state(s) of practice.
- Possesses a minimum of two (2) years' experience in a healthcare facility or equivalent setting.
- CPR certification.
- Demonstrates excellent verbal and written communication skills, as well as strong interpersonal skills.
- Licensed driver with auto insurance that meets state / Agency requirements. Demonstrates a safe driving record and reliable transportation.
