Proficient in nursing assessment skills and OASIS data collection

Description :

Provide care in home health setting.

Work five days a week, Monday to Friday.

Complete an average of six visits per day.

Visits may include rural areas within a specified radius.

Accountability for the care and management of patients.

Conduct nursing assessments and documentation.

Manage peripheral IVs, central lines, and ports.

Coordinate services and discharge planning.

Conduct patient education and support.

Assess and manage various medical devices and conditions.

Observe and report any changes in patient conditions.

Utilize technology effectively in patient care.

Ensure adherence to infection control precautions.

Implement pain management strategies.

Assist with routine visit assessments.

Provide wound care and management.

Document patient care accurately and timely.

Participate in interdisciplinary support as necessary.

Maintain compliance with facility policies and procedures.

Adhere to holiday work requirements.
