The Position :
St. Patrick Center, a ministry of Catholic Charities, is hiring a Mental Health Case Manager, to join the team at Rosati, our off-site location.
This position is responsible for providing individualized counseling, treatment, rehabilitation, advocacy, and case management services to program clients with co-occurring mental health diagnoses and substance use history.
The Mental Health Case Manager utilizes best practices to assess and treat clients while providing proper resources to help assist them in their wellness journey.
This position is expected to work in collaboration with a multitude of teams and services across St. Patrick Center. The core working hours are Monday through Friday, 8 : 00 AM-4 : 30 PM.
