Position Overview :
The Population Health RN (PH-RN) serves as a critical member of the Medically Vulnerable Population (MVP) clinical team.
The MVP is an innovative, multi-partner, respite, and recuperative care facility for individuals experiencing homelessness.
It is designed to promote wellness, deliver equitable care, and address social factors that impact patient's health. The PH-RN will collaborate with community agencies, local hospital systems, homeless resource centers, and other partners to provide coordinated high-quality health management for the individuals temporarily housed there, according to established standards of health care.
Duties & Responsibilities :
- Perform admission and discharge reviews for clients in the MVP.
- Assist with care gap closure, medical evaluation, and chronic disease management.
- Provide front-line nursing care, evaluation, and assessments for clients at the MVP.
- Identifies patient care requirements as part of the triage process and establishes personal rapport with clients.
- Maintains continuity of care among the medical teams by documenting and communicating actions, irregularities, and continuing needs.
- Completes all documentation on time as established in clinic policy.
- Promotes patient's independence by establishing patient care goals; teaching patient, friends, and family to understand condition, medications, and self-care skills; answering questions.
- Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards;
making or recommending necessary adjustments; following Fourth Street Clinic philosophies and standards of care set by the State Board of Nursing, State Nurse Practice Act, and other governing agency regulations.
Maintains a safe and clean working environment by complying with procedures, rules, and regulations; calling for assistance from the health care support personnel when appropriate.
Qualifications and Competencies :
Required :
- The Registered Nurse is a graduate of an accredited program, is licensed in Utah, and is in good standing with DOPL.
- Excellent assessment and organizational skills acquired through previous nurse experience.
- ACLS / BLS certified upon hire or within 90 days of hire date.
- Familiarity with wound care techniques.
- Ability to multitask and prioritize care based on patient needs and clinic flow.
- RN must work well with the clinic team including primary care, behavioral health, dental, front desk, and client services to prioritize care.
Work Environment
The PH-RN's primary workspace is at the Medically Vulnerable Populations facility in Sandy, Utah near public transport hubs.
The facility houses up to 164 clients in 90 units. The working space is a collaborative multiagency environment that provides meaningful patient / client engagement.
Work hours are Monday through Friday 8 : 30 am to 5 : 00 pm.
Preferred :
- Community / Population Health experience
- PHNA-BC or CPH certifications
- Experience working with those experiencing homelessness
