Essential Duties and Responsibilities : Provide initial / reassessment nursing assessments on all clients, following the guidelines set forth by the Department of Health Participate in audits by Agency and / or regulatory bodies.
Ability to create a Plan of Care (POC) that will fit the client’s needs. Conduct a minimum of 23 assessments per week. Ensure timely Start of Care visits in Manhattan and the Bronx.
Travel to the Bronx and Manhattan boroughs, with occasional work in Brooklyn and Queens. Create and adhere to a personal schedule to guarantee timely visit completion.
Confirm visit completion daily and submit visit paperwork within 72 hours. Other responsibilities as assigned. Qualifications and Skills : To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

Must hold a valid NYS RN License Home Care Experience preferred. Excellent communication skills Compassionate, Caring & Understanding Flexibility to accommodate last-minute visit requests.
Bilingual preferred; Spanish-speaking is required. Work Environment : On the Road Pay Range : The hiring range for this position is $110,000.
