Serenity Hospice Care is looking for compassionate and dedicated Licensed Practical Nurses to join our team in New Jersey.
This individual will be responsible for care & services to patients as needed. Candidates must hold a valid LPN License in the state of New Jersey in order to be considered.
This position will be 7PM to 8AM and will be 1 : 1 for patients.
Responsibilities :
- Care for patients in the location as needed.
- Assist with activities of daily living.
- Follow care plans and deliver the highest level of care.
- Document medical information as required.
- Collaborate with interdisciplinary team.
- Communicate with families and patients.
Requirements :
- Valid LPN License in NJ.
- Prior Hospice Experience a Plus!
- Drivers License with Auto Insurance.
- Reliable Transportation.
