Job Description : n nJob Title : Mental Health Therapist Program / Department : Outpatient Reports To : Clinical Director Employment Status : Full-Time Qualified Services : Outpatient Therapy, TBS, PSR, CPST, MHDT PHI Security : Has access to clinical records subject to the limitations as noted in GuidingPoint Policies and Procedures pertaining to Routine Uses and Disclosures of Protected Health Information .

Access is limited to activities as required by job duties and responsibilities. Personnel will disclose immediately to his / her supervisor any potential conflict-of- interest pertaining to clients assigned.

No PHI record will be accessible to the case manager that creates a potential conflict-of-interest as defined in applicable laws and regulations and GuidingPoint policies and procedures (e.

g. records of friends, neighbors and relatives without the proper authorization of the client to access those records). SUMMARY : This position is designed to provide "General services" that include assessment activities, medical activities, and counseling and therapy activities as defined in Ohio Rule 5122-29-03.

The Mental Health Therapist will provide comprehensive behavioral health counseling, assessments, as well as other therapeutic services including clinical case management and consultation for children.

Assists clients and families with skill development, anger management, managing mental, emotional and behavioral symptomatology.

The therapist will work closely with students, teachers, administrators, and other relevant stakeholders to support the emotional and psychological well-being of students.

The Mental Health Therapist commits a designated number of hours per week, aligned with caseload demands. Direct provider of professional clinical services as authorized by the Ohio Department of Mental Health and Addiction Services (OMHAS) including diagnostic assessment, individual and / or group psychotherapy, and treatment of mental and emotional disorders under the direct supervision of an independently licensed clinician.

Must be Licensed in Ohio. This position is expected to work with a multidisciplinary clinical and non-clinical professionals and community agency personnel involved with the client and / or client’s family such as the Judicial System, ODJFS, school, etc.

The goal is to address the (a) client’s needs and (b) collaborate regarding the client’s service outcomes. This position coordinates and monitors medically necessary services to promote continuity of care to (a) minimize the incidence and prevalence of symptoms of mental illness;

b) supports the client to remove barriers to recovery and promote successful tenure in the community; (c) facilitate community integration.

Maintains agency standards of productivity and delivers expected client outcomes while effectively managing time and communicating with service providers and individuals at various socio-economic levels.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. General Services : Treatment / Service Planning : Coordinates and participates in the development of the client’s Individual Service Plan (ITP).

Works with the client to develop a strength-oriented treatment / service plan geared to provide the client with the resources necessary to achieve specific goals.

Includes family and / or significant others throughout as appropriate. Coordinates and / or schedules clients for necessary evaluations and assessments.

Provides or ensures the provision of all necessary Case Management services as identified on the ITP, with the objective of maximizing the client’s independent functioning and productivity.

Reviews the Treatment / Service Plan quarterly in order to respond to emerging client needs / progress. Crisis Stabilization : Offers assistance and support during crisis situations, intervening to stabilize the client as necessary.

Consults with supervisor on crisis stabilization and safety planning. Family and Significant Other Support : Consults with the client's family or significant others as deemed appropriate, fostering a collaborative approach to care.

Individual Interventions, Symptom Monitoring, and Self-Management : Provides ongoing monitoring, support, and training in self-care, social integration, skill-building, and health maintenance.

Identifies and mobilizes, supports responsive to identified needs and helps minimize obstacles affecting financial management, personal development, and performance in school and / or employment.

Engages clients in various activities focusing on relationships with families and others, community, coping skills, interpersonal relationships, and adaptation to home, school, and work environments.

Maintains close contact with clients to monitor their response to services and environment. Procurement and Maintenance of Essential Community Resources : Assists clients in obtaining and retaining community resources including entitlements, housing, vocational training, food, and other basic necessities.

Facilitates access to resources from other agencies and organizations, making referrals, ascertaining linkages, and advocating for services when encountering barriers.

Hospital-Based Interventions : Prioritizes service to clients in psychiatric hospitals, actively participating in discharge planning and early intervention strategies with community follow-up.

Client Self-Determination : Offers clients choices based on their values, personal preferences, and strengths, working collaboratively to empower them in making life decisions.

Assessments : An initial assessment must, at a minimum, include an evaluation of : The presenting problem; The risk of harm to self and others;

The use of alcohol or drugs; The treatment history for mental illness or substance use / abuse; and, A medical history and examination (mental status or physical) A comprehensive assessment expands on the initial assessment and obtains additional information that is required to establish and implement a comprehensive treatment plan, and must be completed within thirty days of the initial assessment encounter.

Counseling and Therapy : Interactions with persons served where the focus is on achieving treatment objectives related to alcohol and other substances;

or the persons mental illness or emotional disturbance. Service involves a face-to-face encounter between a client, group of clients, client and family members, or family members and a behavioral health professional.

Therapeutic Behavioral Services (TBS) and Psychosocial Rehabilitation (PSR) : Provides individualized support or care coordination of healthcare, behavioral healthcare, and non-healthcare services.

TBS are goal-directed supports and solution-focused interventions that may involve collateral contacts and may be delivered in all settings that meet the needs of the individual.

  • TBS service activities include, but are not limited to the following : Consultation with a licensed practitioner or an eligible provider pursuant to paragraph (C) of this rule, to assist with the individual's needs and service planning for individualized supports or care coordination of healthcare, behavioral healthcare, and non-healthcare services and development of a treatment plan;
  • Referral and linkage to other healthcare, behavioral healthcare, and non-healthcare services to avoid more restrictive levels of treatment;
  • Interventions using evidence-based techniques; Identification of strategies or treatment options; Restoration of social skills and daily functioning;
  • and, Crisis prevention and amelioration. PSR service activities include, but are not limited to the following : Restoration, rehabilitation and support of daily functioning to improve self-management of the negative effects of psychiatric or emotional symptoms that interfere with a person's daily functioning;
  • Restoration and implementation of daily functioning and daily routines critical to remaining successfully in home, school, work, and community;

and, Rehabilitation and support to restore skills to function in a natural community environment. QUALIFICATIONS : Bachelor’s OR Master’s degree level LSW, LISW, LPC, LPCC license from the Ohio Counselor, Social Worker and Marriage Family Therapist Board required.

Broad theoretical knowledge in child development, developmental disabilities and psychopathology. One to two years experience in counseling and psychotherapy with children / adolescents and / or adults.

Ability to work with a wide range of resource professionals, psychiatrists, social workers, counselors, and CPST workers.

Must be available to see clients during school hours, day treatment programming and critical times, including evenings and weekends.

Valid Ohio driver’s license, a working vehicle, and current automobile insurance which meets state minimum requirements.

Certification : Able to participate in First Aid, CPR, Verbal / Physical De-escalation; and GuidingPoint Required professional training and development.
