About The Position
Job Title : Physical Therapist Job Specialty : Physical Therapist Job Duration : 13 weeks Shift : Day - 5x8 - 8 : 00 AM - 4 : 30 PM, 40 hours per week Guaranteed Hours : Experience : Minimum 1 year License : Physical Therapist Professional License in Massachusetts Certifications : Not specified Must-Have : 1 year of home care experience Job Description : Provide physical therapy services to patients in their place of residence suffering from physical impairments, functional limitations, or disabilities due to injury, disease, or other causes.
- Collaborate with patients, their families, physicians, and other allied health professionals to identify patient needs and coordinate services.
- Utilize thorough and timely electronic documentation and interdisciplinary communications in accordance with agency policies, procedures, and standards of practice.
- Work with a diverse patient age range from birth to over 100 years old.- Participate in weekend rotations once every six weeks.
- Ability to travel to assigned cases as needed.
