VNA Home Health & Hospice is seeking an organized and timely Registered Nurse Case Manager for Home Health to join our team.
As a pivotal member of our multi-disciplinary team, you will act diligently and respectfully with your patient caseload, ensuring comprehensive and coordinated care.
You will demonstrate exceptional autonomy while maintaining strong connections with Clinical Managers, fellow Case Managers, and other team members to support both patient and team needs.
This full-time position operates Monday through Friday during daytime hours, with coverage transitioning to other case managers in the late afternoons for overnight shifts.

The Work of the Home Health Registered Nurse Case Manager Includes :
- Completes an initial, comprehensive and ongoing assessment of patient and family to determine needs.
- Provides professional nursing care by utilizing all elements of nursing process and as defined in the state Nurse Practice Act.
- IV infusion skills desirable
- Develops a care plan that establishes goals, including the patient and the family in the planning process.
- Regularly re-evaluates patient and family / caregiver needs and revises the plan of care as necessary.
- Uses health assessment data to determine nursing diagnosis.
- Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
- Counsels the patient and family in meeting nursing and related needs.
- Provides health care instructions to the patient as appropriate per assessment and plan.
- Assists the patient with the activities of daily living and facilitates the patient's efforts toward self-sufficiency and optional comfort care.
- Acts as Case Manager when assigned by Clinical Manager and assumes responsibility to coordinate patient care for assigned caseload.
- Communicates with other members of the interdisciplinary team to facilitate optimal care of the patient / family.
Home Health Registered Nurse Case Manager Must Have :
- A valid Missouri and Kansas RN License to Practice.
- Current CPR
- Must be a licensed driver with an automobile that is insured in accordance with state and / or organization requirements and is in good working order
