Hospice RN Case Manager
Miracle City Hospice is seeking a compassionate RN Case Manager to join our dedicated team. This position offers a Monday-Friday 8a-5p shift with occasional on-call days.
Territory coverage includes the East side of Cleveland Lake / Cuyahoga . We offer a comprehensive benefits package and a supportive team environment.
Responsibilities include primary patient and family care, coordination with interdisciplinary teams, and maintaining accurate patient records.
Qualifications include graduation from an accredited RN program, current state licensure, CPR certification, and knowledge of quality assurance procedures.
A valid driver's license and proof of insurance are required.
