SUMMARY : Provides excellent patient care by assisting in the collaboration, development, implementation, revision and reporting of the case management program.

Acts as a liaison between the patient, family, acute care nurse, physicians, multidisciplinary team and patient’s healthcare benefactor to optimize outcomes.

Serves as a consult to the healthcare team on specific patient items. Coordinates a continuum of care for a defined population of patients from admission through discharge.

Assures that patients and families proceed efficiently through the course of hospitalization.

QUALIFICATIONS : To perform this job successfully, an individual must be able to perform each essential function satisfactorily.

HOURS : Monday - Friday, 830a - 530p

Qualifications

DUCATION and / or EXPERIENCE : High School Diploma or GED required. Graduate of an accredited

LVN Program. A.A., Diploma preferred. One to two years related experience and / or training; or equivalent combination of education and experience.

CERTIFICATES, LICENSES, AND REGISTRATIONS : Current California LVN license.
