About The Position
Job Title : Speech-Language Pathologist (SLP) - Home Health Job Specialty : SLP Home Health Job Duration : 13 weeks Shift : Day, 5x8-Hour (08 : 00 - 17 : 00) Guaranteed Hours : Not guaranteed Experience : Minimum of 1 full year of adult home health or hospice experience License : Must have a valid license Certifications : CPR, BLS Must-Have : Proficiency in HomeCare HomeBase (HCHB) used within the last 6 months Job Description : Provide, coordinate, and direct the provision of home nursing care according to physicians' orders based on agency policies and procedures through the competent application of the nursing process.
- Responsible for supervising and evaluating Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.
- Conduct home visits and attend periodic office meetings.- Reliable transportation, a current driver's license, and agency-required liability insurance are required.
- Manage an average of 5-8 patients per day.- Complete documentation and travel activities assigned a point value, with a weekly target of 30 points.
- Types of cases include Start of Care, Wound Care, Follow-up Visits, etc.- Must be open to traveling up to a 75-mile radius from the home clinic;
mileage reimbursed at the IRS standard rate.- Local candidates accepted at the same rate.- Must be proficient in Oasis and HomeCare HomeBase (HCHB) for RN, PT, and OT.
Weekend and on-call requirements vary by location and profession.- Dress code requires scrubs.- The assignment involves skills review.
Note : Applicants must discuss weekend and on-call requirements during the interview and be open to floating to nearby clinics if necessary to meet financial needs.
A retraining course may be required for those whose HomeCare HomeBase proficiency is outside the 6-month period.
