Job Summary

The Breast Services Navigator is a member of a multidisciplinary team who coordinates patient care throughout the continuum in collaboration with Community Radiology, the Breast Center and Precision Medicine and Genetics.

As a clinical resource and advocate, they will ensure timely, efficient, and seamless access for new patients and referring physicians to the breast surgery center.

While maintaining a focus to improve access and the patient experience, the Breast Services Navigator will provide patient education and care coordination from a practice point of entry while managing patient relations and community referrals.

When serving as a counselor for patients at an increased risk of developing Breast cancer, facilitation of patient appointments, including those made with labs, diagnostic areas, and specialty physicians will occur.

The Breast Services Navigator will be responsible for monitoring and evaluating program metrics related to the coordination of services related to screening, diagnostic and preventive imaging services aligned with the COC and NABPC accreditation standards.

The completion of appropriate referrals to social work, patient navigation and financial counseling will remain a top priority while adhering to the Frederick Health Mission and Standards of Behavior.

Required Knowledge, Skills, and Abilities

  • Able to assess and care for special needs patients with respect to cultural, ethical, and religious differences and physical abilities.
  • Must be able to work in a self-directed environment, with the ability to work with and lead teams.
  • Ability to implement professional and community-based education programs with excellent presentation skills utilizing Microsoft Office products.
  • Must be proficient at explaining high risk status and its implication to the patient at a level a patient would understand.

Ability to use and to understand high risk screening models to facilitate referrals to the High-Risk Breast Program and Precision Medicine and Genetics.

  • Ability to serve as a liaison between Community Radiology Imaging departments, Providers within the community and the FH (Frederick Health) Breast Center.
  • Assist in removing any barriers to high-risk care and coordination (i.e., scheduling, insurance etc....).

Coordination of Patient Care

  • Assists patients across the imaging navigation process in understanding of diagnosis, treatment options, and resources available, including educating eligible patients about their high-risk status and the need for genetic assessment along with education about appropriate clinical research studies, technologies, etc.
  • Facilitates appointments for referral and support services within established service standards, including social workers, dieticians, physician, etc.
  • Ensures timeliness of patient diagnosis and treatment through assessment, coordination, and communication.
  • Provides education and information to the patient and designated individuals, helping to make the care seamless, continuous, and comprehensive.

Initiates and documents patient teaching including family and designated others based on assessment of needs. Responds to patient requests for information regarding the disease process, expected side effects of treatment and community resources.

Demonstrates knowledge, skill, and coordination to provide nursing care and guidance to the high-risk patient from screening through regular surveillance process.

A close partnership between Breast Center Oncology Nurse Navigation will occur if patient develops cancer.

Ensures timely communication with referring providers regarding high-risk status, recommended screening protocols, etc.

Accountability

  • Participates in evaluating program effectiveness and takes steps to ensure success in achieving program goals.
  • Initiates and performs ongoing review of policies related to the service provided. Where appropriate, collaborates with Breast Center and PM&G providers to update and / or develop new policies to enhance professional practice.
  • Serves as an essential link between patients and the interdisciplinary healthcare team (physicians, office staff, diagnostic staff, Breast Center providers, PM&G providers, nurses, radiology staff, social services staff, and radiation oncology staff, etc.).

Management of Information

  • Creates and maintains an adequate referral tracking system.
  • Completes all documentation in a timely manner ensuring accuracy and completeness.
  • Maintains legible, accurate and confidential medical records. Documents all medical evaluations, diagnosis, procedures, treatment, outcomes, education, referrals, and consultations appropriately and consistently based on state and federal regulatory standards and in compliance with departmental expectations.

Customer Service

  • Functions as a patient advocate to ensure the patient receives high quality care, delivered to the highest service standards.
  • Consistently concerned with the well-being of the patient. Treats all patients with courtesy, dignity, and respect.
  • Maintains goals established for patient satisfaction surveys.
  • Implements service recovery as needed to promote patient satisfaction.
  • Actively participates in the implementation of organizational patient satisfaction / customer service initiatives.

Teamwork / Community Service

  • Acts as a role model and develops and maintains a collaborative working relationship with the surgical and oncology providers, Precision Medicine, medical staff, nursing, and ancillary departments.
  • Shares knowledge and information as appropriate with various other members of the healthcare team and documents recommendations made utilizing standardized care protocols in accordance with nationally recognized care guidelines.
  • Functions as a resource to other FMH departments regarding care requirements for cancer patients.
  • Serves as a resource for community educational events, such as health fairs, screenings, symposiums, lectures, and staff education.
  • Organizes and supports community awareness and prevention activities for high-risk screening and breast cancer.

Quality Assurance / Performance Improvement

  • Works cooperatively with various departments to improve quality and performance outcomes relating to women’s health issues.
  • Works with Cancer Registry staff and others to collect data, track outcomes, and support strategic planning processes.
  • Maintains documentation for the 27 standards required for the National Accreditation Program for Breast Centers.
  • Maintains quality analysis tools for breast cancer programs.
  • Maintains professional licensure.

Minimum Education, Training, and Experience Required

  • Current licensure as a Registered Nurse in the State of Maryland. BSN preferred.
  • Must have 3 - 5 years of clinical experience as a registered nurse in a hospital or clinic setting.
  • Must currently hold or attain the National Consortium of Breast Centers, Inc. (NCBC) Certified Breast Patient Navigator Examination within one year of hire.

Designation of CN-BN, CN-BI, or CN-BM.

  • Current CPR Certification required.
  • Must maintain licensures and yearly CEU requirements for Certified Navigator, and RN.

Patient Contact

If applicable, must demonstrate and maintain current knowledge and skills in providing appropriate care / contact for patients in the following age groups :

  • Neonate (0 thru 30 days)
  • Infant (31 thru 12 months)
  • Child (13 months thru 12 years)
  • Adolescent (13 years thru 17 years)
  • X Adult (18 years thru 65 years)
  • X Geriatric (66+ years)

If applicable, they must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient’s status.

Possess the ability to interpret the appropriate information needed to identify each patient’s requirements relative to his / her age-specific needs and to provide the care needed as described in the unit’s / department’s policies and procedures.

Physical Demands :

Light Work Lift up to 20 pounds on an infrequent basis (less than one lift every three minutes and / or carrying up to 10 pounds or requiring walking or standing to a significant degree (about six 6 hours a day).

Ergonomic Risk Factors :

Repetition : Repeating the same motion repeatedly places stress on the muscles and tendons. The severity of risk depends on how often the action is repeated, the speed of the movement, the required force and muscles involved.

Forceful Exertion : The amount of physical effort required to perform a task (such as moving patients) or to maintain control of equipment or tools.

The amount of force depends on the type of grip, the weight of an object or person, body posture, the type of activity and the duration of the task.

Awkward Posture : Posture is the position your body is in and its effect on the muscle groups involved in physical activity.

Awkward postures include repeated or prolonged reaching, twisting, bending, kneeling, squatting, working overhead with your hands or arms, or holding fixed positions.

Contact Stress : Pressing the body hard on sharp edges can result in placing too much pressure on nerves, tendons, and blood vessels.

For example, using the palm of your hand to hit or stick objects can increase the risk of musculoskeletal disorders.

Working Conditions :

Blood borne Pathogens Exposure Risk : Category A WILL have exposure to blood or body fluids.

Reporting Relationship :

Reports directly to the Practice Manager while receiving direction from Breast Surgeons, Breast Center APPs, and Precision Medicine providers.

Salary Range : $66,024.19 - $103,542.40
