Hiring a Community Health Nurse. Must be a Registered Nurse (RN).

The Prince George's County Government Department of Health is seeking qualified applicants to fill a fulltime Community Health Nurse IIIposition, grade A24, within its Health and Wellness Division.

Must have an RN License in the state of Maryland or Compact license.

This is a hybrid position.

About the Position :

This is advanced senior / lead level professional staff work performed to address the health care needs of target populations with preventing or managing chronic diseases using the nursing process of assessment, planning, implementation, and evaluation.

Incumbents provide a variety of professional nursing and health care services to a diverse range of clients, patients, families, and groups from diverse cultural backgrounds.

Such services provided by incumbents are designed to promote health and wellness and prevent disease and disability.

The PGCHD works to protect and support the public's health through numerous services that range from restaurant inspections and disease tracking to care coordination and health promotion.

The department is composed of four divisions : Behavioral Health Services, Environmental Health and Disease Control, Family Health Services, and Health and Wellness, supported by the Office of Administration, Office of Human Resources and the Office of the Health Officer.

The mandate of the department has been, and will remain, broad. This plan emphasizes improving core functions and operations, including protecting the public from health threats, implementing strategies to support healthy living, and maintaining a focus on vulnerable populations, while developing efficient internal processes to support these activities.

Examples of Work

  • Administer evidence based chronic disease programs which focus on diabetes prevention and management, obesity, and heart disease
  • Recruit practices, community-based organizations, and pharmacies to participate in various chronic diseases programs.
  • Recruit participants for the program through outreach efforts, partnerships with healthcare providers, community organizations, and pharmacies.
  • Assist practices with re-engineering their workflows to align with timely identification of patient eligibility for participation in the Chronic Diseases programs.
  • Collaborate with pediatricians, dietitians, psychologists, mental health professionals and other healthcare professionals to develop comprehensive integrated treatment plans based on evidence based principles.
  • Work with medical providers to provide input into the criteria for screening workflows and care protocols to drive appropriate screening frequency, early diagnosis, and implementation of care plans.
  • Develop and implement individualized educational plans for patients and their families, emphasizing healthy lifestyle choices, nutrition, and physical activity.

Provide ongoing counseling and support to encourage positive behavior changes.

  • Deliver health educational content about evidence-based quality management through Communities of Practice sessions, teleconferences or webinars.
  • Deliver and develop multi-track learning collaboratives that promote workforce engagement and educational opportunities with partnering practices.
  • Perform health screenings according to established protocols, which may include measuring blood pressure, glucose levels, cholesterol levels, BMI, and other relevant measurements.
  • Refer participants to healthcare providers or community resources for follow-up care as needed based on screening results using muti-directional system Willingly and cooperatively perform tasks and duties which may not be specifically listed in the classification specification or position description, but which are within the general occupational category and responsibility level of the employee's classification of work

Qualifications

  • Graduation from an accredited school, college, university or an equivalent program with a Bachelor's Degree in Nursing or a related health field
  • Two years of post-licensure community health nursing experience
  • License to practice as a Registered Nurse in the State of Maryland.

General Plan Information :

The Prince George's County benefits plan year is from January 1 to December 31.

A spouse (to include a same sex spouse) can be added to the health benefit plans. A marriage certificate and social security number is required to add a spouse.

Children under the age of 26 are eligible for coverage under the health benefit plans. This includes stepchildren and children of the same-sex spouse.

A birth certificate(s) and social security number(s) is required to add a child(ren). If you are only adding the stepchildren or child(ren) of a same-sex spouse, you will need to submit a marriage certificate.

You will also need to submit the birth certificate of the child(ren) and your spouse must be listed as a parent.

The premiums for health benefits are deducted on a pre-tax basis with the exception of Long-Term Disability, Extra Life Insurance and Voluntary Benefits (Short-Term Disability, Whole Life Insurance, Critical Illness, Accident Insurance, Cancer Indemnity, Hospital Indemnity Protection, Accident Indemnity Plan, Supplemental Dental and Group Legal Services).

New employees must enroll in the County's health benefit plans within thirty (30) days of the hire date.

The effective date of the health benefits coverage is the beginning of the month following a waiting period of forty-five (45) days from the date of hire.

After enrolling in the County's benefit plans, employees may only make changes to the plans either during the open enrollment period, which occurs annually (usually each October), and / or during the year, due to a family status change (i.

e., marriage, births, divorce and adoption).
