Summary

The People Concern is partnered with the Department of Health Services (DHS) and the Office of Diversion and Re-entry (ODR) to provide housing and Intensive Case Management Services (ICMS) to persons being released from jail and experiencing homelessness.

Participants are linked to the ODR program while they are in jail and prior to their release. Once released, the ODR team assists the client with obtaining interim shelter and eventually permanent supportive housing in the community.

Participants are linked to an ODR case manager who provides intensive field-based case management and clinical services while employing trauma-informed care and harm reduction approach to a diverse group of clients with complex trauma-facing medical, mental health and substance use barriers.

Participants remain in the program for as long as they require subsidized housing. For many individuals, this may mean their entire lifetime.

Responsibilities

The Program Manager is responsible for the daily oversight of the ODR Team as well as regular coordination with the DHS and ODR multi-disciplinary team, FSP team, probation officer, court, judge, and various interim housing sites.

This includes assistance with client crisis management, coordination of services, and administrative duties. The Program Manager serves as the primary liaison between DHS / ODR and the ICMS team, ensuring funder standards are met.

  • Provides administrative supervision to Bachelor Level Case Managers and Masters Level Clinical Case Managers.
  • Facilitates weekly team meetings and case consultation utilizing a trauma-informed lens and harm reduction approach.
  • Responsible for coordination of clients' release from jail which includes ensuring that transportation is available, interim housing is arranged, substance use treatment is in process, Medi-Cal has been activated, and all medications have been ordered.
  • Attends court hearings with clients and case management staff to advocate and ensure appropriate clinical recommendations are made on behalf of the client.
  • Oversees submission of weekly court progress notes which includes gathering information from the multi-disciplinary team, reviewing for errors, and submitting the final version by court deadline.
  • Acts as primary liaison between client, case management team and various interim housing sites which may include emergency shelters, substance use treatment facilities, and board and care.
  • Collaborates with the ODR psychiatrist, DHS multi-disciplinary team, probation officer, judge and community partners to develop creative interventions focused on improving the safety and well-being of our most marginalized clients.
  • Assists with client crisis management which includes assessment for psychiatric hospitalizations.
  • Responds to emergency calls and situations on a 24-hour basis with support from ODR Clinical Supervisor and ODR Director.
  • Ensure all documentation, charting and data collection is complete, timely and accurate through ongoing audits of electronic databases and chart reviews.
  • Reviews case managers' progress notes on a weekly basis to ensure funder and billing expectations are being met.
  • Provides assistance with hiring and staffing ODR team and ensuring staff are adequately trained and abide by all policies and procedures.
  • Ensure case managers are linking clients to appropriate services including FSP, other mental health resources, substance use resources, medical, vocational and benefits and entitlements.
  • Participates in monthly calls and meetings with DHS and community partners.
  • Perform other duties as assigned.

Qualifications

  • MSW, MFT, LPC or PsyD (supervision hours are available for this position) preferred, or 3 years of equivalent experience.
  • Minimum of 2 years' of management experience, including administrative and clinical supervision of staff.
  • Extensive experience working with the homeless population or individuals with severe and persistent mental illness.
  • Knowledge of the criminal justice system plus.
  • Able to work in a high tolerance Harm Reduction model with clients who have multiple barriers.
  • Current, valid California Driver's with an acceptable driving record.
  • Able to be on-call 24 / 7 with support from ODR Clinical Supervisor and ODR Program Director.

Preferred Qualifications

MSW, MFT, LPC or PsyD (supervision hours are available for this position) preferred.

Work Environment

  • Field (may need to travel) and indoor office environment.
  • On occasion walk or drive to different local sites.
  • Regularly required to sit, stand, bend and occasionally lift or carry up to 35 pounds.
  • Will necessitate working in busy and loud environments.
  • Will be exposed to elements like cold, heat, dust, noise and odor.
  • May need to bend, stoop, twist, and sit throughout the day.
