🩺 Job Responsibilities
You will serve as an adjunct to the Hospice Medical Director , providing advanced care, face-to-face patient evaluations, and clinical coordination. Major duties include :
🔹 Clinical Assessments & Hospice Eligibility
Conduct face-to-face visits in patients’ homesEvaluate appropriateness for home health or hospiceEnsure recertification and ongoing eligibility for hospice
🔹 Coordination & Interdisciplinary Work
Collaborate with :Hospice Physicians, Social Workers, Dieticians, Volunteers, Spiritual Care, Therapy Services
Participate in IDG team conferencesCommunicate plan-of-care updates and changes to teamHelp transition patients smoothly into hospice
🔹 Other Key Duties
Perform hospice admissions and document findingsSubmit weekly visit schedules and documentation per policyMeet productivity standards and clinical competencyUse agency-provided devices (tablet, phone, etc.) and document using point-of-service tech
🎓 Qualifications
✅ Active Nurse Practitioner License (California)✅ Minimum 5 years of RN experience✅ At least 1 year of recent RN experience or BSN from an NLN-accredited school✅ Current California Driver’s License & Auto Insurance✅ CPR Certification (AHA recommended)✅ Must be proficient with electronic documentation and mobile devices✅ Strong self-direction, flexibility, and team collaboration skills✅ Prior experience in program development and teaching preferred✅ Must meet clinical competencies per Pathways guidelines
