🩺 Job Responsibilities

You will serve as an adjunct to the Hospice Medical Director , providing advanced care, face-to-face patient evaluations, and clinical coordination. Major duties include :

🔹 Clinical Assessments & Hospice Eligibility

Ensure recertification and ongoing eligibility for hospice

🔹 Coordination & Interdisciplinary Work

Hospice Physicians, Social Workers, Dieticians, Volunteers, Spiritual Care, Therapy Services

Help transition patients smoothly into hospice

🔹 Other Key Duties

Use agency-provided devices (tablet, phone, etc.) and document using point-of-service tech

🎓 Qualifications

✅ Must meet clinical competencies per Pathways guidelines
