Summary Description : Greater Mental Health's Health Home Serving Children Program is committed to providing community-based care management services to children ranging from birth to 21 years of age.
As a Care Manager (CM) on our team, you will play a crucial role in coordinating the development and execution of individualized, community-based plans encompassing social, emotional, and medical services for children and adolescents with special needs.
Key Responsibilities :
- Provide outreach and engagement to children and families.
- Conduct a comprehensive, strengths-based assessment of the child and family and their service needs.
- Develop a child and family specific plan of care in collaboration with each family and their care team.
- Assist families in securing identified services and advocating to other systems for the provision of these services in an individualized and collaborative manner.
- Facilitate the creation of a support network for each child and family; convene regular meetings with the family and care team to monitor progress towards jointly developed goals.
- Facilitate ongoing communication among the child, family and community providers.
- Provide crisis intervention and stabilization to children and families as needed.
- Develop knowledge of and positive relationships with the multiple systems serving families and youth in Westchester County.
- Provide 24-hour telephonic crisis intervention services on rotating basis.
- Document client progress and maintain a permanent record of client activity according to established methods and procedures.
- Practice with a person-centered, trauma informed philosophy in alignment with the values of Greater Mental Health.
- Due to the nature of the program, must have the ability to be flexible with working hours.
- Complete face to face contacts and telephonic care management services based on the individual’s needs.
- Adhere to all standards and guidelines established by applicable regulatory authorities.
- Other duties as assigned.
Required Knowledge and Skills :
- In-depth knowledge of behavioral health diagnoses and symptomatology.
- The Care Manager must complete the CANS-NY Training and be certified to use the CANS-NY upon hire.
- Proficient in effectively engaging clients.
- Exceptional organizational skills and self-motivation.
- Familiarity with and ability to use an Electronic Health Record system.
Requirements :
- Bachelor’s Degree with two years of relevant experience, or licensed as a Registered Nurse with two years of relevant experience, or a Master's Degree with one year of relevant experience.
- Basic computer knowledge is required.
- Reliability and the ability to work independently, with a strong understanding of the importance of maintaining confidentiality.
- Possession and maintenance of a valid Driver's License with a satisfactory driving record according to our insurance carrier's standards.
- Must maintain current NYS Defensive Driving certification within 30 days of hire.
- Must maintain pre-employment clearances by applicable regulatory clearances as required.
- Legal eligibility to work in the United States.

- Salary Range : $44K- $50K, based on departmental budget, qualifications, experience, education, and licenses. This range represents the agency’s good faith and reasonable estimate of the range of possible compensation at the time of posting.
