Registered Nurse - Primary Care, Crown Heights
One of the NY metro's largest multi specialty Physician practices as an immediate need for a Registered Nurses to work in Primary Care.
Specialty Registered Nurse are responsible for providing primary care to patients, including assessment, treatment, Charting, care planning and medical care evaluation.
Type : Contract
Schedule : Mon-Fri, 40 hours a week not inclusive of their breaks
Location : Crown Heights
Responsibilities :
Reviews work to be completed and develops a plan based upon patient / population need, skill levels of other nursing team members, ACP policy / procedure and applicable rules and regulations.
Supports implementation of programs and procedures developed at an organizational level.
Collects data and adjusts process to accommodate program requirements.
Maintains licensure and certification as required and is responsible for participation in ongoing education activities.
Teaches and supports patients with self-care management.
Actively seeks input to support patient care and outcomes.
Participates in program evaluation activities.
Follows up on abnormal laboratory results requiring patient action and possibly re-measurement (increasing medication dose, dietary changes, etc).
Handles requests for medication refills according to ACP procedure.
Coordinates outpatient work-up and or treatment plans when patient situation requires it (e.g. determination of diagnosis, facilitates communication between physicians and health care providers).
Provides direct care for patients needing blood pressure check, wound check, technique assessment.
Monitors patients while awaiting transport to Emergency Room and assures documentation is complete.
Participates in the development of strategies to maintain high levels of wellness for at-risk populations.
Practices in accordance with nursing standards and organizational policy following established procedures for infection control, patient care and safety that are in compliance with all federal, state and local standards (OSHA, HIPAA, etc.).
Provides direct patient care, as appropriate, including, but not limited to wound care, medication administration and accessing venous access sites.
Qualifications :
Completion of nursing training from an accredited school of nursing
Minimum of one to two years' experience working in a Medical Practice / Primary Care, Med Surg, ER and / or Occupational Health
Current New York State Registered Nurse license
