Care Coordinator (Part-Time)

Families & Youth Innovations Plus (FYI+)

Position : Part -Time (up to 28 hours); Hourly / Non-Exempt

Salary : $21.63 - $26.18 hourly (3 levels based on experience and education)

Reports To : Sandra Ortega, Family Services Clinical Manager

Department : Comprehensive Addiction and Recovery Act (CARA)

Job Summary

The FYI Plus Care Coordinator provides specialized care coordination services for infants and families affected by substance exposure through the Comprehensive Addiction and Recovery Act (CARA) program. This position focuses on providing non-punitive supportive care for pregnant people affected by substance use and coordinating comprehensive services for parents, caregivers, and family members of newborns affected by substance exposure. The role combines hospital-based identification and engagement with community-based ongoing care coordination, emphasizing that substance use in pregnancy should not, by itself, be grounds for child protective services involvement.

What You'll Do :

  • Conduct daily rounds to identify eligible NICU and mother / baby unit patients.
  • Engage with hospital staff to obtain Medicaid enrollment lists.
  • Ensure Plans of Safe Care (POC) are completed and submitted.
  • Provide education and resources to birth mothers and legal guardians.
  • Perform Health Risk Assessments (HRA) and Comprehensive Needs Assessments (CNA).
  • Develop and implement Comprehensive Care Plans (CCP).
  • Coordinate medical, behavioral health, and social services.
  • Monitor ongoing care coordination and facilitate disease management.
  • Engage community agencies and follow up on referrals.
  • Support hospital discharge planning and long-term care coordination.

What You Need :

  • Bachelor's degree in social work, nursing, healthcare administration, psychology, or related field
  • Experience in healthcare case management, care coordination, or maternal / child health services
  • Knowledge of substance use disorders, family systems, and their impact on families
  • Understanding of trauma-informed care principles and non-stigmatizing approaches
  • Proficiency in electronic case management systems
  • Valid driver's license and reliable transportation for hospital visits and community engagement
  • Strong communication skills and the ability to coordinate complex care plans across multiple service providers and agencies
  • Experience with perinatal substance use and addiction recovery services
  • Bilingual capabilities (English / Spanish) preferred
  • Some experience with family-centered care coordination
  • Special Requirements :

  • Maintain confidentiality and follow specific CARA reporting protocols
  • Comply with all MCO, state, and federal regulations regarding care coordination
  • Participate in ongoing training related to CARA program requirements and substance use disorder treatment
  • Ability to provide a non-stigmatizing, equitable response to perinatal substance use
