Description
The Nurse is responsible for coordinating and overseeing all health care services including health education, required by the residents of the Children’s Community Residence (CCR).
The Nurse will ensure all medical and clinical needs of each youth are maintained in accordance with Office of Mental Health and Hygiene guidelines.
The Nurse will evaluate and recommend policies and procedures related to the regulatory requirements of the CCR program.
The Nurse will coordinate with local vendors (pharmacy, medical providers, etc.) to ensure all medical and mental health needs of CCR residents are maintained.
The ideal candidate should be able to maintain a flexible schedule, working 12-16 hours weekly, be proactive and be able to communicate effectively.
As the medical personnel within the CCR, be able to operate through that lens and provide clear medical directions to the team.
Responsibilities :
- Respond to monthly supervision with Program Director and monthly supervision with Medical Director.
- Monitor and manage medication for the assigned CCR residents including arrangement of medication for weekend visits, which includes packing medication for youth’s absence from the residence.
- Follow up timely on semi -annual Physician Authorizations.
- Coordinate on all new admissions to ensure timely establishment of medication refills.
- Meet with all residents on a monthly basis to check in on their overall health and well-being, medication management etc.
Notes should be added to Precision Care.
- Oversee and supervise all direct care staff administering medication.
- Provide proper training of direct care staff in the administration of medication. Training should be done within the first week of employment and quarterly thereafter.
- Maintain clear documentation of all training, records etc.
- Communicate with family / guardian about all medical conditions, appointments, and / or treatments.
- Engage family to accompany child / with residence staff on medical appointments.
- Accompany and / or transport resident to medical appointment when appropriate.
- Oversee the general health care of the resident including basic first aid, arrange for appropriate medical and dental visits.
Provide follow up care as needed ensuring that medical treatments that need to be provided in the residence occur as ordered and are documented as required.
- Assess any injuries, recommend treatment, or follow up and document accordingly while notifying residence, parent, and guardian.
- Provide health education to residents and staff, including prevention, medication side effects, nutrition, management of chronic health problems such as asthma, diabetes, sex education (age appropriate), substance abuse, COVID safety protocols, and other medical concerns where applicable.
- Teach residents the importance of basic needs, handwashing, hygiene, oral and body, CPR and First-Aid, self-concept Skills related to smoking, drugs, and alcohol usage.
- Coordinate with identified pharmacy to ensure medication is available consistently.
- Monitor and manage the Medication Administration Records (MARs).
- Monthly assessments and identification of medical needs.
- Coordinate care plans with family, medical providers, and community agencies including school personnel.
- Assess the youth’s medication management goal and establish plans with youth where appropriate to develop self-administration of medication.
- Safeguard CCR resident right to privacy by protecting information of a confidential nature.
- Initiate referrals for vision and hearing, dental and or children’s medical services.
- Engage in professional development affiliated with role of the RN position.
- Collaborate with CCR team related to health concerns of residents.
- Participate in community and staff meetings arranged at CCR site.
- Participate in case consultation when applicable.
- Ability to manage multiple priorities, coordinate high level tasks, and manage report request when required.
- Other assistance and support as requested by Residence Manager and Program Director.
- Interact with Paycom Database in accordance with job expectation. Correspond and collaborate with identified service providers by phone, fax, and email.
Qualifications
Qualification :
- Bachelor's degree from four-year college or equivalent. Master's degree or equivalent preferred
- 5 or more years’ experience in the nursing field with the focus on the adolescent population, knowledge of trauma, medication management, ability to manage multiple priorities, ability to work as a team member.
Excellent communication skills in engaging with youth and their families.
