The Licensed Social Worker LCSWCrisis &Triage Management provides ambulatory behavioral health servicesincluding evaluation diagnosis treatment planning case managementand psychotherapeutic intervention provided to individuals familiesgroups and / or Intensive OutpatientPrograms.

EssentialResponsibilities :

• Assistswith administration development implementation and evaluation ofsocial work programs andpolicies.
• Work as part of a team inthe Call Center Advice Line to provide telephonic triage andassessment for members requesting behavioral health services tofacilitate the appropriate level ofcare.
• Provide telephonic crisismanagement for members seeking behavioral health services.
• Coordinate admissions and dischargesfor members requiring psychiatricadmissions.
• Provide telephonic followup for post hospital and ERdischarges.
• Conducts clinical socialwork consultation. Performs clinical diagnosis risk assessments andcrisis interventions.
• Providespsychotherapy and counseling for individuals families groups and / orIntensive Outpatient Programs. Utilizes a wide range of evidencebased interventions and modalities.

Provides services such ascrisis intervention brief therapy cognitive and behavioraltherapies skills training supportive therapy stress management andfamily therapy.

Interviews member andevaluates their psychological and mental status. Utilizes thelatest Diagnostic and Statistical Manual of Mental Disorders (DSM)to diagnose patients.

Determines if member is in crisis atrisk orsuicidal / homicidal. Refers members in need of immediate or urgentcare to appropriate level ofservices.

Assesses urgent or highriskclinical situations and makes appropriate interventions.Coordinates referral of patient to psychiatrist or other clinician.

Assists with admission to hospital or other specialized facilities.Plans and provides services in the appropriate level of care.

Assesses needs for mental health treatment and makes appropriatereferrals as needed.Performs case management coordination andmonitoring of social work service plans in areas of personal socialor economic resources conditions orproblems.

Assesses treatment andprevention of psychosocial dysfunction disability or impairmentincluding emotional and mental disorders.

Conducts preparation andevaluation of psychosocial assessments and development of socialwork service plans.

• Helps patientsenhance or restore their capacities for personal and socialfunctioning and preventing and controlling socialproblems.
• Gathers information andidentifies resource and referral services for patient care.Educates client regarding behavioral health services .

Assistsmembers to use services.

• Participatesin or assists with therapeutic and educational group programs.Evaluates outcomes and effects of treatment for individual patientsand programs.
• Researches collects andrecords data and information relating to identification andtreatment of behavioral and interpersonal problems of patient.

Proposes alternatives for interventions and approaches forresolutions.

Collaborates andconsults with physicians and other interdisciplinary clinical teammembers to support assessments and decision making.

Reviewsdifficult cases or quality assurance concern with clinical team.Acts as consultant to team members and others in theorganization.

Maintains high level ofclinical expertise. Is knowledgeable of current clinical conditionsmethods medications theoretical frameworks and interventions.

Utilizes best practices in delivery of behavioral health servicesin accordance with professional and clinical guidelines.

Utilizesevidence based interventions and modalities with sound theoreticalbase.

• Maintaining clinical records bydocumenting clinical assessments treatment plans interventions andall pertinent information in patient medical record according toRegional and departmentalguidelines.
• Participates in clinicalteam meetings departmental planning and quality managementactivities.
• Assesses patients inEmergency Departmenets on after hours and as needed basis inservice areas where Kaiser Permanente has priviledges to providemental health assessments and crisisintervention.
• Other clinical dutiesas assigned.

Requirements

Education

MastersDegree in social work from an accredited college or universitypassing score on the clinical level national board examination andsupervised clinical social work training as required by HawaiiRevised Statues (HRS) 467E7.

License CertificationRegistration

• LicensedClinical Social Worker (Hawaii)
• National Provider Identifier required athire
• Basic LifeSupport required at hire from American HeartAssociation

AdditionalRequirements :

• Knowledgeand experience in conducting assessments and diagnostic evaluationsusing the latest Diagnostic and Statistical Manual of MentalDisorders (DSM).
• Demonstratedknowledge of and skill in adaptability change management conflictresolution decision making group process facilitation influenceinterpersonal relations oral communication problem solving qualitymanagement teamwork and writtencommunication.
• Taxonomy code requiredat time of hire.
• Note : This positionrequires successful completion of credentialing process uponhire.

PreferredQualifications :

• Demonstratedknowledge and skill in documentation according to nationalaccreditation guidelines.
• Experiencein providing behavioral health services in a primary caresetting.
• Experience in providingservices to patients with addictionsissues.
• Experience in providingConsultation / liaison Services to emergencydepartments.
• Experience in providingevidence based treatment including conducting individual and grouptherapy intensive outpatient treatment and / or partialhospitalization programs for children and / or adults includingpeople with serious / chronic mentalillness.
• Experience in writingtreatment plans.
• The positionrequires working over the phone in highly stressful situations andwith interruptions.
