What you'll do
The Licensed Practical Nurse, Charge will work Full-time, Evenings and the primary purpose of this position is to evaluate and direct nursing care of residents on a particular nursing care unit.
Integrate the philosophy and objectives of the Nursing Home and the Nursing Department into the management of nursing care.
The Licensed Practical Nurse, Charge initiates plan of care :
Assign resident care in accordance with resident need and to the abilities of the staff members assigned to the unit.
Observes the residents physiological and psychological behavior and notifies the NCC of any concerns and documents observations in the resident’s medical records..
Provide for continuity of care for all residents on assigned unit and cooperate in providing for continuity of care for all three (3) shifts.
The Licensed Practical Nurse, Charge provides updates regarding the plan of care to residents, families, nursing staff and other nursing home personnel.
Participate in resident teaching programs.
Contribute to resident care plans, defining needs and concerns of residents.
Discuss administrative and nursing care problems with Nurse Care Coordinator and / or DON.
The Licensed Practical Nurse, Charge reviews all physicians orders on a daily basis and insures they have been programmed accordingly.
Responsible completion of a Quarterly Nursing Summary for each resident on assigne unit.
Participates in Quality Assurance activities as directed
Keep informed of new medical and nursing measures as related to the needs of residents.
The Licensed Practical Nurse, Charge maintain environment conducive to providing quality care.
Make rounds to observe effectiveness of care and consult with Nurse Care Coordinator to make changes when necessary.
Give care to residents who require skilled nursing as ordered.
Demonstrate new nursing techniques per facility policies.
Maintain a system for communication of nursing care information for each resident in the unit.
The Licensed Practical Nurse, Charge make rounds with physicians, as needed.
Participate in nursing and resident care conferences.
Maintain nursing documentation in accordance with facility policies and procedures.
The Licensed Practical Nurse, Charge use nursing care plan as a guide for reports, rounds, conferences and assignments.
Share information with other para-medical groups and professional disciplines.
Review critical incidents and discuss with Nurse Care Coordinator / DON.
Assure responsibility as directed by the Nurse Care Coordinator for teaching relative to planning, implementing and evaluating the nursing care for each resident on the unit.
The Licensed Practical Nurse, Charge observe nursing care requirements relative to the number of residents, severity of conditions, medical treatment plans and nursing care activities and report concerns to the Nurse Care Coordinator.
Assign personnel in accordance with resident needs and technical competency of personnel.
Identify problems of nursing care and in assignment of personnel.
The Licensed Practical Nurse, Charge participate in In-service programs.
Participate in orientation programs.
Supervise and counsel direct care personnel assigned to unit.
Create and maintain working climate conductive to producing quality care.
The Licensed Practical Nurse, Charge assure direct care staff work assignments are completed daily.
Respect individual for ability, knowledge and skills.
Plan opportunities for growth and development.
Guide working relationship with residents, families, visitors, nursing personnel, medical staff, other departments and representative of community agencies.
Support personnel policies.
The Licensed Practical Nurse, Charge recommend changes to Nursing Supervisors / DON.
Keep personnel informed of changes.
Interpret new, or current and standing policies for staff members.
Review all incident reports and discuss prevention with staff.
Maintain an effective communication system on unit.
The Licensed Practical Nurse, Charge report any personnel problems to Nursing Care Coordinator.
Follow Standards of Practice and facility polices and protocols for medication and treatment administration.
Responsible for the narcotics cabinet, the maintenance of records, the receipt and use of narcotics when assigned as med nurse.
Support new programs and policies.
Suggest policy changes to Nurse Care Coordinator / DON.
The Licensed Practical Nurse, Charge responsible for notifying the appropriate discipline for the maintenance or upkeep of equipment and supplies on the unit.
Check emergency equipment every week, i.e., O2, oral suctioning, ambu bags, CPR board to insure good working order and report concerns to Maintenance and Nursing Administration.
The Licensed Practical Nurse, Charge assure that nursing unit is clean neat and functional (dirty utility room, clean utility room & kitchen).
Insure each resident is treated with dignity and respect and report any concerns with resident care and treatment to Administration in accordance with facility policy.
Must follow the established infection control procedures.
The Licensed Practical Nurse, Charge participate in In-service education programs and in continuing education programs designed to keep abreast of changes in the nursing profession, keep LPN license current and provide a copy for personnel file.
Adheres to facility Code of Conduct including a commitment to professionalism and service excellence.
Assist in all other duties deemed necessary by Supervisor. This includes providing care and assisting with residents as needed.
You may be required to work on other shifts or units as needed to maintain acceptable staffing levels.
The Licensed Practical Nurse, Charge attend all required in-services and follow all department and facility policies and procedures including the use of personal protective equipment.
Maintain CONFIDENTIALITY of all pertinent resident care and employee information and follow HIPAA and HIV regulations.
What we need from you
Education :
Must be a graduate from an accredited school of nursing and current licensure by the State of New York as a Licensed Practical Nurse
Experience :
One (1) year experience as a LPN in which potential supervisory competence has been demonstrated, preferred
License / Certification :
Must be a Licensed Practical Nurse, required

Pay Transparency
Salary is based on a variety of factors, including, but not limited to, qualifications, experience, education, licenses, specialty, training, and fair market evaluation based on industry standards.
