Oak Medical is committed to providing consistent, high-quality care across a range of medical environments. We are seeking a dedicated Senior Living Internal Medicine Nurse Practitioner (NP) or Physician Assistant (PA) to join our professional team supporting facilities in the Waukesha area.

Under the clinical supervision of licensed physicians, you will deliver diagnostic and therapeutic health care to patients in Skilled Nursing, Memory Care, Assisted Living, and Independent Living environments.

Your role will involve obtaining, evaluating, and interpreting lab results, performing physical exams, and carrying out therapeutic procedures.

Schedule : Monday Friday. No nights, no holidays, no on-call shifts

Key Responsibilities :

• General Medical Care : Provide comprehensive medical care to adult and geriatric patients, addressing both acute and chronic primary care complaints.
• Evaluations : Conduct thorough evaluations including history taking and physical examinations to establish and document accurate medical diagnoses.
• Diagnostic Testing : Order and interpret relevant diagnostic laboratory and radiology tests to support medical diagnoses and treatment plans.
• Medical Planning : Develop and implement individualized medical plans of care based on patient needs and clinical findings.
• Procedures : Perform diagnostic and therapeutic procedures within your level of training and expertise.
• Medication Management : Prescribe medications as appropriate and manage medication regimens for optimal patient outcomes.
• Documentation : Ensure timely and accurate documentation of patient care, including billing and medical records.
• Collaboration : Work collaboratively with supervising physicians and accept feedback and direction to enhance patient care.

Qualifications :

• Education : Certified Nurse Practitioner (NP) or Physician Assistant (PA) with WI state licensure.
• Valid Wisconsin DEA
• Experience : At least 1 year of experience in internal medicine, geriatrics, or similar fields is preferred.
• Skills : Strong diagnostic and clinical skills, excellent communication abilities, and proficiency in performing medical procedures.
• Attributes : Compassionate, detail-oriented, and capable of working effectively in a team-oriented environment.
